Hair flap surgery is an older hair restoration technique that involves lifting a strip of hair-bearing scalp, rotating it, and placing it into a bald area while simultaneously removing the balding skin. This method was developed before the advancement of modern hair transplant procedures like FUE and FUT.

Hair flaps were primarily used to create instant hair coverage in large balding areas, particularly in cases of frontal baldness. The advantage of this procedure was the immediate density it provided, as the transplanted strip contained a high volume of hair follicles. However, hair flaps had several drawbacks, including unnatural hairline positioning, scarring, and reduced blood supply to the rotated flap, which could lead to complications like necrosis (tissue death).

While hair flaps were once considered a viable option, they have largely been replaced by more refined techniques that offer better aesthetic results with minimal scarring. Today, hair transplant surgeons focus on creating a natural-looking hairline using individually transplanted follicular units rather than large scalp flaps.
